The designations of benefit recipients to be made shall continue through and after termination of employment by the Employer.
C. Designation
The Designation is made pursuant to Title 10, CMJ. Pursuant to Department of Defense Directive (Dodd) 5165.09, entitled “Department of Defense Individual Pay, Allowances, and Benefits for Service members,” dated June 1, 2001, entitled “Pension Benefits for Military Retirees,” dated May 29, 2001, and applicable to the military department for which the Service member is serving, and subject to Dodd 5165.09, Title 10, CMJ, it is the desire of this employee, (hereinafter referred to as “the Designated Person”) that this designation includes his or her immediate immediate immediate family as follows: spouse (if applicable) and dependents.
D. Notification of Beneficiary Selection
Any payments provided by the Employee Designation to the Designated Person shall be deposited directly into an American Express® Checking Savings Account opened pursuant to the instructions in Form 4442, and remitted to the Account Holder and deposited into the Account Holder's financial institution of choice within the timeframes specified in the instructions on the form. All payments made pursuant to the Designation shall be deposited into an American Express® Checking Savings Account within thirty-one (31) days of the date the payments are due or as otherwise specified in the instruction on the form.
E. Form of Designation
The Designation of Beneficiary is a written, legible signature acknowledging receipt of the form and indicating that the Designated Person has read the instructions, the information in Part 5, Paragraphs 1-5 and Part 5, Paragraph 10, concerning his or her designations, and is consenting to and has made an affirmative agreement to accept the benefits and other payments of the Employee Designation.
